We design each patient's care plan to complement physician orders and provide further support with evidence-based disease management and our staff's clinical expertise. We offer Skilled Nursing, Behavioral/Psych Nursing, Physical, Occupational and Speech Therapy, Wound Certified Nurse, education for medication, nutrition, and self-care management, Home Health Aides, Medical Social Worker, and coordination of medical equipment. Our Right Path Programs include: Cardiac Care, COPD, Diabetes, Joint Rehabilitation, Late Life Depression & Palliative Care. We are in 31 states across the nation. We are Medicare certified & accept most insurance plans. We locally service Carbon, Lehigh, Monroe & Northampton Counties along with most of Southeastern PA. In addition, we have added services up north in Columbia, Lackawanna, Luzerne, Lycoming, Mountour, Northumberland, Pike, Snyder, Sullivan, Union, Wayne and Wyoming counties.
