Patients undergo a thorough assessment performed by a specialist educated in geriatrics. The assessment includes a review of their medical and social history, medications and cognitive testing. During the assessment, a specialized treatment plan will be recommended and communicated to patient's caregiver, primary care office and other specialists. Our team focuses on the caregiver as well.
